(ex-reference number 32178) of Toronto, registered since September 11, 2003 had its renewal of registration refused due to: failure to indemnify Tarion in excess of $11,000.00; failure to provide security; failure to execute and deliver the document detailing specific Terms and Conditions of Registration. |

David Frenette of Hanover, had his application for registration refused due to the following: insufficient equity; failure to provide a Personal Net Worth Statement; failure to provide back up documentation for Personal Net Worth Statement; failure to provide security; failure to execute and deliver the Vendor and Builder Agreements; acting as a vendor and/or builder without being registered; failure to enroll; conviction under section 22 of the Ontario New Home Warranties Plan Act; failure to provide reference letter from applicant’s financial institution. |
